Our clinical team conducts comprehensive assessments in collaboration with individuals, their families, carers and existing healthcare professionals. This enables us to understand each person’s individual needs and develop a safe, appropriate and person-centred care package, with clear care plans and risk assessments supporting the delivery of their care.

Health Now provides individualised care packages for young people over the age of 13 and adults, including those aged over 65, who require community-based support for physical health, mental health, cognitive or multiple complex needs. We can support individuals with co-existing conditions and multiple diagnoses, subject to our clinical assessment and acceptance criteria.

Through our clinical expertise and person-centred approach, we can provide safe and compassionate support for individuals with higher-level, complex and multiple care needs, helping them to live as independently and comfortably as possible.

Nursing-Led Care

We provide nurse-led care packages tailored to the individual clinical and personal needs of each person we support. Our Registered Nurses provide clinical oversight, helping to ensure care is delivered safely, effectively and in accordance with agreed care plans and clinical requirements.

Our nurses undertake comprehensive assessments, develop and review care plans and risk assessments, and provide clinical guidance and supervision to the care teams supporting each individual. We work collaboratively with families, healthcare professionals and the wider multidisciplinary team to provide coordinated, person-centred care.
